We require the documents listed below:
  1. W9 form
  2. Carrier Profile
  3. Insurance certificate including:
    1. General Liability of at LEAST $1 Million
    2. Auto Liability of at LEAST $1 Million
    3. Risk Cargo of at LEAST $100,000
    4. Statutory Work Comp & $500,000
  4. Hazardous Material Registration (if applicable)
  5. Authority form
  6. Signed Carrier/Broker Agreement
Can I get a dedicated lane?

Paramount does offer both EDI and ACH.
 
How do I check my payment status?
Please call 1-877-564-4789 (LOGISTX). Please have Paramount’s PRO# available. This is available on the Rate Confirmation sheet.
 
Paramount Logistics’ Payment Terms
We pay net 30 days from receiving complete carrier invoice paperwork, which includes our PRO #, the rate confirmation, and the signed Bill of Lading along with your invoice.
